The State of Indiana, Shelby County, SS
            The undersigned, applicant for letters of Guardianship on the persons and estates of Emma Williams aged 3 years in 1879; James A. Williams aged 2 years in 1879, minor heirs of James V. Williams, late of Shelby County, Indiana; said minors reside in Shelby County, in the State of Indiana.
            Said wards also owned personal property of the value of One Hundred Dollars.

            Matthew Sedgwick being duly sworn, says that the material facts contained in the above statement and application for Letters of Guardianship on the persons and estates of the persons therein named are correct and true, as he verily believes, and further saith not.

To the Honorable the Judge of the Circuit Court of said County.
            The undersigned, Matthew Sedgwick, guardian of Emma and James A. Williams, Minor heirs of James V. Williams, deceased, would respectfully represent to said Court, that he is chargeable with the assets of said wards as follows, to-wit:
            Dec 17th 1879 Received from Fred H. Chueden, Clerk, the share of said wards of the estate of Daniel Williams, deceased (great-grandfather) $36.18

            Said guardian further shows that he has paid out on account of said wards the following sums for which he should have credit to-wit:

            To Elizabeth Bassett (mother) on support of said wards                     $25.49
            To Fred Chueden, Clerk                                                                           2.69
            To Matthew Sedgwick guardian services                                                   8.00
            Total disbursements                                                                              $36.18


            He further shows that the above is a correct amount of all receipts and expenditures and that no other assets have come to his knowledge.
            He asks the court to accept this report and release him from further trust as guardian of said wards.

            To the Honorable, the Judge of the Circuit Court of said county, Benjamin H. Boon Respectfully represents to the court that on the 5th day of May 1878 James V. Williams died in said county, leaving a wife and minor children, leaving no real estate, and personal property worth not over five hundred dollars and this he asks may be vested in said decedents widow.
